If you have not signed up for the portal, please contact us by phone or in person and we will be happy to help you become a Portal access patient! To make an appointment, please call us or contact us through the form below.
You will need to enter your user ID and Password to login to the Patient Portal. Request your prescription refill or appointment and a response will occur either through the Portal or by one of our staff.
You can also see your lab and test results along with visit summaries on the portal. The portal can also be used to send us demographic changes or let us know you need to reschedule or change an appointment.
Parking is available in the Sacred Heart Doctor’s Building on a rate per hour basis as determined by Providence Sacred Heart Medical Center.
Parking is also available in open lots across the north and south streets and by metered parking on the streets south of the building. Our office validates you but not your parking.
The Sacred Heart Doctors Building parking’s current policy is to accept cash and credit cards only – no debit cards and no checks. There is an ATM machine and a Washington Trust Bank branch in the lobby of the building for your convenience. You pay the garage attendant on your way out.
